Facial analysis and planning consultation for filler and botulinum toxin, illustrative image Illustrative image
In brief

Botox temporarily relaxes expression muscles to soften expression lines; filler supports volume loss, contour and facial proportions. They answer different problems and do not replace each other. They are often combined and can be given in the same or in separate sessions; which treatment, in what order and whether on the same day is decided after the facial analysis. An international consensus report describes combined treatment as a widely used approach and states the goal as facial harmony rather than rejuvenation.

Many people thinking about botulinum toxin (Botox) or fillers in Mersin start with the same question: "Filler or Botox?" Then comes a second one: "Can I have both at the same time?" In this article I explain what each treatment addresses, how they are planned together, whether they can be done in the same session or separate ones, and what to look for when choosing a doctor.

Filler or Botox? What Does Each One Address?

Botox is effective for dynamic lines that come from muscle movement: forehead lines, the lines between the brows and crow's feet belong to this group. Hollowing caused by volume loss, deepened nasolabial folds or a faded cheek and jawline are not corrected by relaxing a muscle; in that picture a filler that supports volume is the more suitable option. The two do not replace each other; they answer different problems. This is a general summary; timings vary with the person, the area and the product: For crow's feet see our guide to crow's feet Botox, and for the cheeks our cheek filler guide.

If your concern falls outside what Botox can address, Botox is not recommended, and if there is no volume loss, a filler is not recommended. Not recommending a treatment you do not need is the basic principle of a consultation. You can find the details of both treatments on our botulinum toxin and filler pages.

Can Filler and Botox Be Done at the Same Time?

Yes, it can be done. Filler and Botox are often combined and can be given in the same or in separate sessions. An international consensus report also describes combined treatment as a widely used approach; where volume loss affects muscle activity it recommends keeping the toxin dose low enough to modulate the muscle rather than paralyse it, and focusing on the aesthetic units of the face rather than isolated lines. The objective it stresses is facial harmony rather than rejuvenation (Sundaram et al., 2016).

That said, there is not a separate clinical study for every product combination; for example, the US Food and Drug Administration (FDA) states that the safe use of dermal fillers together with Botox and other treatments has not been evaluated separately in clinical studies (FDA, 2023). So the question "can it be done on the same day?" has no single answer for everyone; the decision is made according to the person and the plan.

Same Session or Separate Sessions?

In combined plans the two treatments can be given in the same session or split into separate ones. Which route is suitable is decided together after the facial analysis, according to the areas to be treated, the scope of the plan and your preference. The two treatments take effect at different speeds: Botox starts working in 3–5 days and its full effect appears in 10–14 days; the look of a filler is largely visible immediately and its final result settles in 1–2 weeks.

Because of this difference, working in two stages in some plans can make it easier to assess the result of each treatment separately. When more than one procedure is done on the same day, it is also taken into account that swelling and small bruises may appear in the same period.

Which Concern Calls for Which? Examples of Combined Planning

  • Jawline: A feeling of width in the lower face is not always caused by lack of volume. When the chewing muscle (masseter) is prominent, jawline filler on its own may not give the definition you expect; an integrated plan combined with Botox may give a more balanced profile. For jawline and chin filler see our jawline guide, and for masseter Botox our masseter Botox guide.
  • Lips and upper-lip lines: For fine lines on the upper lip, a small amount of Botox is often used together with lip filler. We explain the difference between lip filler and lip Botox (lip flip) on our lip filler page.
  • Expression lines and volume loss: Forehead and between-the-brow lines are addressed with Botox, and the hollowing and sagging feeling in the midface with filler; the two can be part of the same treatment plan.
  • Skin quality: Treatments aimed at skin quality, such as mesotherapy, may also be added to the plan for dullness and loss of moisture. Recommending every procedure to everyone is not the right approach.

If They Are Done Together, How Is the Plan Built?

  1. 1Examination and health history: Conditions that affect suitability, such as pregnancy and breastfeeding, medicines you take, allergies and neuromuscular diseases, are reviewed.
  2. 2Facial analysis: Your expressions, the areas with volume loss and your facial proportions are examined together. The images from the Face Simulator software used in the preliminary consultation are illustrative; they do not show the real result and are not a promise of an outcome.
  3. 3Goal: A goal is set that looks at harmony across the whole face rather than at individual lines or areas.
  4. 4Areas, product and amount: Which treatment will be done in which area, how much and the name of the product are discussed.
  5. 5Order and timing: Whether the two treatments will be done in the same session or in separate ones, and which will come first, is decided together.
  6. 6Follow-up: Because the full effect of Botox appears around days 10–14, the check-up is planned for that period; the final result of a filler settles in 1–2 weeks and small corrections are made if needed.

Safety: Which Side Effects Can Occur?

With Botox, common and temporary side effects are redness at the injection points, tenderness, small bruises and a mild headache in the first days; less often, a temporary droop of the eyelid or brow and asymmetry may occur. With fillers, swelling, bruising and tenderness are common; palpable firmness, infection and allergy are less common; and a filler entering a blood vessel is rare but the most serious risk. For a review that covers the possible side effects of both, see the sources section (Anais Brasileiros de Dermatologia, 2025). Choosing a Filler and Botox Doctor in Mersin

  • Who is performing the treatment: Is the person a medical doctor, and are their name and authorisation clear?
  • Product: How can you be sure that the Botox is licensed and the filler is registered in ÜTS?
  • Plan: Is a separate rationale given for each treatment, and is nothing you do not need being recommended?
  • Follow-up: Is there a check-up visit and a way to reach the clinic if there is a problem?
  • Price: Is it clear whether the price covers the product, the amount and the follow-up?

Frequently Asked Questions

Can filler and Botox be done on the same day?

Yes, filler and Botox are often combined and can be given in the same or in separate sessions. Which route is suitable is decided after the facial analysis, according to the areas to be treated and the plan.

Is filler or Botox better?

Neither is better than the other; they address different problems. Botox softens lines that come from expression muscles, while filler supports volume loss and contour.

Which comes first, filler or Botox?

There is no fixed order; the order and timing are decided at the facial analysis according to the plan. Because the full effect of Botox appears around days 10–14 and the final result of a filler settles in 1–2 weeks, the check-up visit is important.

Can lip filler and Botox be done together?

For fine lines on the upper lip, a small amount of Botox is often used together with lip filler. Whether volume or a slight definition of the upper lip is needed is decided at the examination.

Does everyone need both filler and Botox?

No. If your concern falls outside what Botox can address, Botox is not recommended, and if there is no volume loss, a filler is not recommended; no procedure you do not need is recommended.
