After lip filler, swelling, redness, bruising, tenderness and a mild feeling of firmness are common; most settle within a few days, bruising can last up to 1–2 weeks in some people and the final shape of the lips settles in 1–2 weeks. In a meta-analysis of randomised trials, side effects were common and serious ones rare (about 1%). Unusually severe pain, whitening or coolness of the skin, a net-like purple-blue discolouration or a change in vision may suggest a vascular complication and is an emergency.
What you see in the mirror after lip filler is often not the final result. Swelling, redness and bruising are common; some of them are expected and some call for letting the physician know. This article is written to give a practical framework for the question "Is this normal?" We described the treatment itself and the healing process day by day in our article on lip filler in Mersin.
Common and Usually Temporary Effects
In a meta-analysis of randomised trials, side effects of lip filler were common and serious ones rare; the most frequently reported were swelling, a feeling of firmness, bruising and tenderness (Wen et al., 2026). The US Food and Drug Administration (FDA) also notes that side effects such as swelling and bruising occur shortly after injection and many resolve in a few days to weeks (FDA, 2023). The lips are among the areas where swelling and bruising are common.
| Symptom | When and for how long, usually? | What to do? |
|---|---|---|
| Swelling | Most noticeable in the first days; usually settles within a few days | A cold compress can be used (do not apply ice directly); avoid intense exercise, sauna, steam bath and excessive heat for the first 48 hours |
| Redness and tenderness | Seen in the first days | For the first 24 hours do not press on the lips or rub the area |
| Bruising | May be more marked in some people; can last up to 1–2 weeks | Wait it out; do not wear makeup on the lip area for the first 24 hours |
| Mild feeling of firmness | Among the frequently reported side effects | If it persists, or if you notice a palpable firmness or irregularity, it is assessed at an examination |
| Feeling of asymmetry | Because of swelling in the first days the look is not final; the final shape of the lips settles in 1–2 weeks | Wait at least 2 weeks to judge the result; if needed a correction is planned 2–4 weeks later |
Situations That Need Attention
If any of the following is present, contact the clinic without waiting a few days and without losing time:
- Herpes (cold sore) flare-up: In people who have had cold sores before, the treatment can trigger a flare-up; mention your history at the preliminary consultation.
- Palpable firmness, a nodule or irregularity: Among complications reported after lip filler, nodule formation is the most common; migration and discolouration have also been reported (Diwan et al., 2023). This is assessed at an examination.
- Signs of infection: Increasing redness and pain, discharge or a sore; infection is a rare side effect.
- Signs of an allergic reaction: Itching, a rash or widespread swelling; allergic reaction is rare.
Emergencies: Signs of a Vascular Complication
Accidental injection of the filler into a blood vessel is rare but the most serious risk; it can lead to loss of skin tissue, stroke or loss of vision (FDA, 2023). One review notes that the earliest symptom of vascular occlusion is pain and the earliest signs are coolness, blanching of the skin (which may be transient) and a net-like appearance, followed within minutes by delayed capillary refill, within tens of minutes to hours by a blue-grey discolouration and within days by breakdown of the skin (JMIR Dermatology, 2024). If any of the following is present, contact the treating physician immediately and go to an emergency department:
- Pain: Unusually severe or steadily increasing pain.
- Skin colour: Whitening or coolness of the skin, or a net-like purple-blue discolouration.
- Vision: A change in vision, double vision or loss of vision.
- Signs of a stroke: Drooping of the face, difficulty speaking, or sudden dizziness or confusion.
In this situation a rapid response is decisive, because a vascular complication needs urgent treatment and hyaluronic acid filler can be dissolved with the enzyme hyaluronidase. Dr. Doğan Uysal's clinic keeps hyaluronidase, and in the event of a vascular complication a rapid response with hyaluronidase is planned.
What to Do and Not to Do in the First Two Weeks
The recommendations are listed by time; personal instructions are shared separately after the treatment:
| When | Recommendation |
|---|---|
| First day | Avoid very hot or very cold food and drinks; to reduce swelling use a cold compress rather than applying ice directly. |
| First 24 hours | Do not press on the lips and do not wear makeup on the lip area. |
| First 48 hours | Do not massage the area vigorously; avoid intense exercise, sauna, steam bath and excessive heat. |
| During healing | Reduce salt intake; sodium can increase swelling. |
| First 2 weeks | Postpone a dentist visit for at least 2 weeks. |
| After week 2 | The final shape settles in 1–2 weeks; at least 2 weeks are allowed before judging the result and, if needed, a top-up is planned 2–4 weeks later. |
